Mission

EquaCare’s mission is to prevent the need for emergency care, hospitalization, and nursing home placement among those most at risk—frail elders, the physically or mentally disabled, the chronically ill and the dying. EquaCare empowers organizations, professionals, families, and long-term care consumers, regardless of the care receiver’s economic status, to better plan the care, to use available resources more effectively, and to access community help more readily.
